Go to the Education tab on the left panel. Next, open Spiritual Assessment and select Questionnaires.
Select the checkbox next to the form name. An option called "Archive" will appear in the top bar. Click on the option.
When you click the "Archive" option, a confirmation pop-up appears. Click "Yes" to successfully archive the questionnaire.
