Go to the Education tab on the left panel. Next, open Spiritual Assessment and select Questionnaires.
- Select the checkbox next to the Questionnaires name. An option called “Delete” will appear in the top bar. Click on the option.
- When you click the Delete option, a confirmation message will appear asking whether you want to delete the questionnaire. Click "Yes" to confirm and permanently delete the questionnaire and also remove all question sets and questions that are linked up with it.
